FORBICI PIZZA RESTAURANT – FEEDBACK FORM PROMOTION TERMS &
CONDITIONS

1. INTRODUCTION
Welcome to Forbici! We value your feedback and want to reward our guests for sharing their thoughts. By sharing your feedback you help us improve our customer experience and product quality. As a reward for providing your feedback, you have a chance to be entered into a prize draw, conducted monthly, to win one (1) free pizza at Forbici (the “Prize Draw”), you agree to the following terms and conditions.

2. ELIGIBILITY
– The Prize Draw is open to all guests who have dined at Forbici and completed the official feedback form provided via a QR code on your receipt.
– The minimum age to participate is 18 years old.
– A smartphone, tablet, computer, or laptop with a secure and stable internet connection is essential for participating in the Prize Draw.
– Employees of Forbici, their immediate family members, and anyone directly connected with the Prize Draw are not eligible to participate.

3. HOW TO ENTER
– To enter, must complete the feedback form provided by Forbici after dining with us to automatically qualify for the Prize Draw.
– Only one entry per person per visit to Forbici is permitted.
– Incomplete, inaccurate, or fraudulent entries will be disqualified.
– You can participate in the Prize Draw only once per month.

4. PROMOTION PERIOD
– The Prize Draw is performed once per month and accepts entries till 23:59 of the last date of that month (the “Entry Period”).
– For the monthly Prize Draw, one random winner is selected for the prize of one (1) free pizza at Forbici.
– The Prize Draw for each month is performed on 28 th of the month following the survey entry.
– Entries outside the Entry Period or which do not meet the entry criteria will not be considered

5. PRIZE DETAILS

– The prize consist of one (1) free pizza at Forbici, provided through a giftcard.
– The prize is non-transferable, non-exchangeable, and cannot be redeemed for cash.
– The prize is subject to availability and must be redeemed within the timeframe specified on the digital gift card.

6. WINNER SELECTION &NOTIFICATION
– The winner will be chosen at random from all eligible entries received each month.
– The winner will be notified via the contact details provided in the feedback form within 2 working days of the Prize Draw taking place.
– If the winner does not respond within seven (7) days of notification, a new winner may be selected.

7. DATA PROTECTION & PRIVACY
– By entering, participants consent to Forbici collecting and processing their personal data for the purpose of administering the Prize Draw.
– Personal data will not be shared with third parties, except as required by law.
– Forbici will comply with all applicable data protection laws in handling participant information.

8. GENERAL TERMS
– Forbici reserves the right to amend, suspend, or terminate the Prize Draw at any time without prior notice.
– Forbici is not responsible for technical errors, lost or delayed entries, or any circumstances beyond its reasonable control.
– Any disputes regarding the Prize Draw will be resolved at Forbici’s sole discretion.
– Forbici accepts no responsibility for any damage, loss, injury, or disappointment suffered by any entrant as a result of entering the Prize Draw or accepting the prize.
– The prize draw and these terms and conditions will be governed by English law and any disputes will be subject to the exclusive jurisdiction of the courts of England and Wales. FORBICI PIZZA RESTAURANT – LOCAL FRIENDS PROMOTION TERMS & CONDITIONS

1. INTRODUCTION
At Forbici, we’re excited to share our love of pizza with our local business community! As part of our exclusive Local Friends Promotion, selected businesses will receive complimentary pizza to try, along with a special bounce-back card (the “Bounce-back Card”) to use on a future visit. By using a Bouce-back Card, you agree to the following Terms and Conditions.

2. ELIGIBILITY
– The Bounce-back Card is available only to businesses that have received the Forbici Local Friend promo card directly from Forbici.
– The Bounce-back Card is for individual use and is limited to one voucher per person.
– The Bounce-back Card is valid only when dining with a table of at least two (2) people.
– Employees of Forbici, their immediate family members, and individuals involved in the Bounce-back Card’s distribution are not eligible to redeem the Bounce-back Card.

3. PROMOTION DETAILS
– The Bounce-back Card entitles the bearer to two (2) free snack bites when dining in at Forbici and purchasing at least two (2) pizzas.
– The Bounce-back Card can be used until 31st April 2025.
– The Bounce-back Cardholder must purchase all other items on the bill, excluding the two (2) free snack bites.
– The Bounce-back Card is non-transferable, non-exchangeable, and cannot be redeemed for cash.
– Only one Bounce-back Card can be used per person per visit.

4. HOW TO REDEEM
– The Bounce-back Card must be presented to the Forbici staff before placing an order.
– The Bounce-back Card is only valid for dine-in customers; takeaway and delivery orders are not eligible.
– The table must consist of at least two (2) paying dine-in customers.
– Forbici reserves the right to verify eligibility before honoring the Bounce-back Card.

5. GENERAL TERMS
– Forbici reserves the right to modify or cancel the Bounce-back Card at any time without prior notice.
– The Bounce-back Card is subject to availability, and Forbici may limit redemptions at its discretion.
– Any misuse of the Bounce-back Card may result in refusal of service.
– In the event of any dispute, Forbici’s decision will be final and binding.
